Open Door earns national recognition second year in a row

Open Door Health Services has been recognized for the second year in a row by the U.S. Health Resources and Services Administration (HRSA) as a National Quality Leader in Diabetes Health. Open Door is 1 of 4 health centers in Indiana to receive this designation.

Managing chronic conditions like diabetes requires teamwork, consistency, and trust between patients and providers. This 2026 Community Health Quality Recognition (CHQR) badge is proof of the hard work happening every single day across the Open Door clinics.

This recognition highlights the efforts of our providers and patients for meeting or exceeding the national targets in:

  • BMI (Body Mass Index) screenings and follow up plans

  • Weight management through good nutrition and activity for children and teens

  • Keeping blood sugar (A1C) levels under control

HRSA, Open Door’s primary federal partner, awards these honors annually to recognize health centers showing exceptional performance and commitment to accessible, high-quality care.
