Sports Physicals | $25

To receive a physical:

• Bring completed IHSAA form.

• Parent/legal guardian must be present.

• $25 will be due at time of service for all physicals.

Cash, credit card, and HSA / FSA cards accepted. Insurance will not be billed for physicals at the event.

No appointment needed | Open to the public

Physicals provided by Open Door Health Services. Open Door staff will be onsite to schedule follow-up appointments and/or well-child visits, if needed.

Open Door Hartford City Family Practice is located in the former IU Health Blackford Hospital ER space.
