Immigration Physicals

Open Door offers immigration physicals, a requirement for applying for a green card or adjustment of status. Immigration physicals are offered at our Anderson Family Practice on Tuesdays from 10 a.m.–3 p.m. The immigration physical requires two separate appointments, described below.

Spanish-speaking interpreters; phone interpretation for other languages.

  • Physical exam

  • Vaccination

  • Lab tests (off-site)

    • First appointment

    • Review paperwork

    • Order labs (blood tests)*

    • Determine which vaccines are needed**

    Payment of $400 will be collected upon arrival. This cost includes both the first and second appointments, but it does not include vaccine costs or lab costs.

    • Physical exam

    • Vaccine administration

    **The cost of vaccines will depend upon which vaccines are needed. Pricing can be provided during appointment.

    • Please bring printed blank form; if you do not have access to a printer, Open Door staff will provide a form.

    • Report of medical exam

    • Vaccine record

    • Government-issued photo identification, such as a valid passport or driver’s license

    • For patients 14 years or younger, bring identification that shows child’s name, date and place of birth and parent’s full name

    • Possible identification includes birth certificate with English translation or affidavit

    • Payment of $400 (cash or credit card) due at first appointment
