• Payment of $400 will be collected upon arrival. This cost includes both the first and second appointments, but it does not include vaccine costs or lab costs.

• Please bring printed blank form; if you do not have access to a printer, Open Door staff will provide a form.

• Report of medical exam

• Vaccine record

• Government-issued photo identification, such as a valid passport or driver’s license

• For patients 14 years or younger, bring identification that shows child’s name, date and place of birth and parent’s full name

• Possible identification includes birth certificate with English translation or affidavit

• Payment of $400 (cash or credit card) due at first appointment

• $400 | Due at the time of first appointment
