This position will perform a variety of patient care activities to provide clinical support and assistance to the nurse practitioner. Assists in the delivery of primary health care and patient care management.

This position leads the coordination of data collection, development and analysis of report and development of the agency quality assessment plan.  This position will provide training and support for the electronic health record. Work cooperatively with the providers and clinical staff to assure electronic health record system is used to maximize quality measures and activities to support them are met or exceeded.

This position serves as a Competent Professional Authority (CPA), and is responsible for developing and providing the Nutrition Education Program as well as nutrition counseling.

Performs a variety of patient care activities to provide clinical support, teamwork and assistance to physicians, nurse practitioners, and medical staff. Will assist in the delivery of family medicine, obstetrics and pediatrics, including patient care management. 

Performs a variety of patient care activities to provide clinical support and
assistance to dentists and dental hygienists. Assists in the delivery of dental care and patient care management.

Performs a variety of patient care activities to provide clinical support, teamwork and assistance to physicians, nurse practitioners, and medical staff in regard to Behavioral Health and organizes and implements behavioral Health Services to patients and families. 

This position will perform a variety of patient care activities to provide clinical support and assistance to the nurse practitioner. Also, this position will provide accurate and effective interpretation of procedures and other interactions for non-English speaking patients, their families, and staff (in person or on the telephone).

Will assist in coordination of the enrollment and outreach programs.  Assist in coordination of all activities and enrollments for Hoosier Healthwise, Healthy Indiana Plan, Medicare Part D, SNAP, TANF, Medicaid Disability Assistance Program and federal/state marketplace exchange applications.  Maintain that all clients receive information about possible assistance programs that can optimize billing for the health center. Assist in planning and organizing patient and community outreach activities and coordinate implementation of such activities.

Works with patients, family members, and care team to create a customized Care Plan and coordination of care along the social and healthcare continuum for patients with complex needs.  Provides professional nursing care for primary care patients by following established standards, practices, and protocols.

Assess, evaluate and prioritize urgency of medical needs through telephone communication with patients following established standards, practices, and protocols.
