This position will be responsible for oversight and coordination of performance improvement activities assuring Open Door Health Services (ODHS) improvement activities are successful. Develop, manage and integrate a comprehensive Performance Improvement program to achieve stellar results in quality, efficiency, safety, satisfaction and value with transparency.
This position provides clinical support and assistance to physicians, nurse practitioners, and medical staff. Assists in the delivery of primary health care and patient care management. Responsible for operating X-ray equipment for assisting providers with diagnosing and/or treating disease/injury.
Under direction of the Floor Supervisor of Primary Care this position is responsible for maintaining pharmacy program including ordering, maintaining and tracking supplies through the PHS Drug Pricing Program and the Assistance program.
Performs a variety of patient care activities to provide clinical support and assistance to physicians, nurse practitioners, and medical staff.
Completes needs assessments with patients to identify any gaps, barriers or unmet needs that prevent the patient from achieving an optimal level of health. Works with patients and care team members to create a customized action plan and identify short and long term goals to address barriers, needs and gaps as identified by the needs assessment.
